Appointments and Calls
Please call us at (408) 356-2345 to schedule an appointment, or if you have any questions or concerns regarding your skin condition, treatment, or medication. If we are not available to speak to you directly, we will return your call as soon as possible. If you need to reschedule an appointment, we ask that you let us know 24 hours in advance. This allows us to give that time to another patient. In the event of an after hours emergency, our medical answering service is available 24 hours a day to assist you.

Office Hours
Our office is open Monday through Friday and many Saturdays. Office visits are by appointment only.

Insurance
Our goal is to make the insurance billing process as simple as possible for you. Consequently, our itemized service slip includes all of the necessary information to file an insurance claim. We also bill directly to Medicare and many PPO's, so we ask that you provide complete insurance information at the time of your visit. This includes an insurance ID card, group and plan numbers, and a claims billing address. Although we may bill your insurance for your convenience, you are responsible for your bill. If your treatment is not covered by your insurance, or if we do not receive payment from your insurance company within 90 days, we will bill you directly.

Fees & Billings
Services may be billed to your insurance. If we are not contracted with your insurance, you will receive an itemized service slip, indicating the diagnosis, treatment, and fees. These fees are due at the time of service and can be paid for by cash, check, Visa, Mastercard, or American Express.
